Primary intraosseous squamous cell carcinoma of the jaws (PIOSCC) is reportedly a very rare lesion, but may not be as rare as commonly believed. The source of the epithelium giving rise to an intraosseous carcinoma is believed to be the epithelium involved in odontogenesis. A case of PIOSCC of the mandible initially misdiagnosed as an odontogenic cyst is presented here. The first treatment consisted of enucleation as for a cyst. However followed a histopathological diagnosis of PIOSCC, chemoradiation was the treatment modality prescribed. The literature on carcinomas arising in the jaws is briefly reviewed.
